The claim forms can be found here. Once the forms are filled out, they must be notarized and mailed to this address, with forms postmarked by Sept. 12:

– Family members of the 49 deceased victims killed in the club.
– Survivors who were physically injured and hospitalized for one or more nights between June 12 and Sept. 12 due to injuries from the mass shooting.
– Survivors who were treated for their injuries at an Orlando-area hospital on an emergency out-patient basis and were released without an overnight hospital stay.
– Survivors who were present at Pulse during the mass shooting but did not suffer physical injury requiring medical treatment, based on proof of their physical presence from law enforcement records.
OneOrlando plans to start distributing the funds to approved claimants on a rolling basis starting Sept. 27.
